Tick identifications for residents of New Hampshire only are provided free of charge by the New Hampshire Department of Agriculture, Markets & Food, Division of Plant Industry. We will identify what species the submitted tick is and whether or not it is a species that is a carrier of Lyme Disease. We do not, however, identify whether or not the submitted tick is positive for the Lyme disease pathogen. To submit a tick for identification please fill out the form and either mail the form with the specimen to the Division or bring it to the New Hampshire Health and Human Services Building in Concord (29 Hazen Dr.) and leave the specimen and paperwork in the Department of Agriculture identification drop box (the staff at the front desk can direct you). Care should be taken with the specimen to ensure it arrives in a condition that enables identification. If sending the sample by mail, place the specimen in a vial or crush-proof container.
